Support From Sentencing Through Reentry
Course Connection helps eligible people prepare for release and connect with treatment, recovery services, healthcare, benefits, and other community resources. Case management can begin during incarceration and continue through the early transition back to the community, based on program eligibility, facility participation, and available funding.
About Course Connection
Course Connection is an affiliated nonprofit organization of Allied Correctional Services. Its application for recognition as a tax-exempt organization under Section 501(c)(3) of the Internal Revenue Code is pending.

The program coordinates practical next steps before and after release. A case manager works with each participant to identify priorities, create a goal plan, make referrals, and follow up on barriers that can interrupt care or recovery.
Why The Transition Period Matters
Release can interrupt medications, behavioral healthcare, substance use treatment, medical follow-up, housing, transportation, benefits, and family support. People leaving custody may also face a heightened risk of overdose. Early planning and active follow-up can improve the chance that a participant reaches the next provider and stays engaged.
How The Program Works
1.
Engage. The case manager meets with an eligible participant during incarceration when the program is available.
2.
Plan. The participant and case manager identify healthcare, treatment, recovery, benefits, housing, transportation, identification, and other priorities.
3.
Connect. Course Connection coordinates referrals, appointments, information, and approved community partners before release whenever possible.
3.
Follow up. The case manager checks progress after release, helps address barriers, and supports transition into ongoing community services for the period established by the program.
Eligibility
Course Connection serves eligible people incarcerated in participating Ohio county jail programs. Eligibility, enrollment timing, the length of follow up, and available services vary by facility, contract, grant, and funding source.

Connection to primary care and behavioral health providers
Substance use treatment and medication-assisted treatment linkage
Medication access and continuity coordination
Recovery support and peer service referrals
Naloxone and overdose-prevention education or resource coordination
Medicaid benefits and identification resource navigation
Housing food transportation employment and phone access referrals
Family and approved support person information
Post-release check-ins reminders and referral follow-through

Course Connection can supplement a facility's release planning by coordinating participant engagement, referral follow through, community provider communication, and program reporting. A county specific model may include defined referral criteria, service timelines, partner roles, and outcome measures.
